| summary | Chronic pain is often associated with perceptual, sensory and blood flow disturbances. Historically, such disturbances have been attributed to ‘bottom-up’ mechanisms – facilitation of the neural mechanisms that subserve nociception and pain, and problems with the tissues of the body part that hurts. In fact, until recently, the idea that top-down mechanisms may also be at play has been confined to descending modulation of nociception. My talk will discuss experiments that test the idea that top-down mechanisms contribute to some of the other problems we see in people with pain. We hypothesise that disturbances of higher order cognitive representations, such as the representation of peripersonal space, bodily awareness and body ownership, modulate pain, blood flow and local immune responses. I will present published data and works in progress. |
